Financial Payment & Insurance

Forward Health Members

​

Please be aware that dental services covered by Forward Health may require co-payments. Monetary amounts are pre-determined for each individual according to their insurance plan provided by Forward Health. 

 

Co-pays must be paid at time of check-in. Please be prepared to pay up to $10.00 for each scheduled appointment per family member. We accept cash, checks and major credit cards. 

 

Please bring your Forward Health insurance card along with you and please arrive 15 minutes before your scheduled appointment time to check-in.

It is the policy of Dental Studio to not discriminate in the provision of services to an individual because the individual is unable to pay; because the payment for those services would be made under Medicare, Medicaid, or the Children’s Health Insurance Program; or against any individual on the basis of race, color, religion, ancestry, national origin, citizenship status, age, sex, marital status, genetic information, sexual orientation, gender expression, gender identity, non-disqualifying disability or veteran/military status in matters of admissions, employment, or services.  Discounts are available based on family size and income.  All staff members are fully committed to function in accordance with all applicable federal, state and local laws regarding non-discrimination.

No Insurance? No Worries ...

Payments for main services if your earnings land within the 125% (or below) Income Level 

 

$25.00 for an emergency visit (1 x-ray will be included) 

$30.00 for a comprehensive exam (includes x-rays)
$35.00 for a prophylaxis appointment (cleaning)
$35.00 per tooth that needs filling

$40.00 per tooth that needs extraction

 

Monthly income that is over the 150% Income Level will be charged accordingly of the standard fee. 

 

* Payment arrangements can be made with our Patient Financial Representative

* Each individual is allowed 1 emergency visit. The requirement of having a comprehensive exam will be enforced.

DENTAL SPECIAL SERVICES DISCOUNTS  (Minimum of 5% down of total procedure before scheduled appointment)

 

Additional 5% Discount (if paid in full before initial procedure appointment) OR arranged payment plan may be arranged. 

 

Full amount of procedure is required before final appointment is to be made. Payment plans can be arranged.
